This person will receive payroll email reminders and any important forms coming due

Pay frequency, payroll beginning and ending dates, payroll pay date. Also, were employees paid in the same year? If so, provide past payroll information for accurate year-end filings

Pay frequency will either be weekly, bi-weekly, semi-monthly, and monthly.

If you, your accountant, or even a previous owner of your business paid employees, provide the payroll information to make sure the W-2s you provide to each employee are accurate

We will provide a link, after submission, to drop employees' form W-4, pay rates/salaries, hire dates, birth dates, job titles, work locations, & employee bank information (if direct deposit). Also include any health and retirement deductions/contribution

Do any owners employ their parents or children. If yes, list the employee names

We will provide a link, after submission, to drop business bank information, if direct deposit. Will there be a different bank account for paying payroll withholdings/taxes? If yes, please provide

We will inquire about any pending filings/payments for which we'll be responsible, to date payroll reports, and log-in information for federal, state, and local withholding/taxes

Any PTOs provided to the employees? If so, provide ending balances. Also provide accrual rates and whether it carries over to the next year

Provide ONLY when paying employees through direct deposit. Lists of all people who own 25% or more of the business AND the number of years you've been in business
